By submitting this form, I authorize Ferguson Accounts to deposit my pay directly into the account(s) indicated. I understand and agree to the following:

This authorization will remain in effect until I submit a written request to cancel or update my direct deposit information.

I am responsible for ensuring that the account information provided is accurate. Ferguson Accounts is not responsible for delays or losses due to incorrect or incomplete banking information submitted.

I understand that verification of my bank information may require a pre-note (test transaction) and that my first payment may be issued by paper check if necessary.

If funds are deposited in error, I authorize Ferguson Accounts to initiate debit entries to correct the transaction.

Any changes to my banking information must be communicated at least 2 business days before the next payroll processing date to ensure timely updates.
